What is Polyvagal Theory?
Polyvagal Theory, developed by Dr. Stephen Porges, describes the autonomic nervous system as having three hierarchically organized states — ventral vagal (safe and social), sympathetic (mobilized — fight or flight), and dorsal vagal (immobilized — freeze or shutdown) — and explains how trauma and chronic stress disrupt the system's ability to access the ventral vagal state of safety.
Dr. Stephen Porges, Distinguished University Scientist at Indiana University, developed Polyvagal Theory through his research on the vagus nerve — the tenth cranial nerve, the longest in the body, connecting the brain to every major organ. He identified that the vagus nerve has two distinct pathways with different evolutionary histories and different functional roles.
The ventral vagal state is the newest evolutionary development, unique to mammals. It is associated with safety, social engagement, curiosity, and connection. When the ventral vagal system is online, a person can read facial expressions accurately, modulate voice prosody, tolerate closeness, and recover quickly from stress.
When safety is perceived to be absent, the system drops to older strategies: the sympathetic nervous system mobilizes for fight or flight; and if mobilization is impossible (inescapable threat), the dorsal vagal branch — the most ancient, shared with reptiles — produces immobilization, freeze, dissociation, and shutdown.
The clinical application is significant: many trauma symptoms that look like "psychological" problems — inability to trust safety, chronic hypervigilance, social withdrawal, dissociation — are actually autonomic nervous system states. They cannot be resolved through cognitive insight alone but require nervous system-level interventions that build the capacity to access and sustain the ventral vagal state.
